AB Occupational Therapy
AB Occupational Therapy respects the privacy of all clients. This document outlines our privacy rights and practices, as guided by the Privacy Act 1988 (Commonwealth) and Information Privacy Act 2000 (Vic). These privacy laws regulate how personal information is obtained and handled, from its collection and use to future disclosures, as well as how it’s stored and when it’s disposed of. You can get a copy of the Australian Privacy Principles from The Office of the Australian Information Commissioner’s website at https://www.oaic.gov.au/.
Personal Information Collection
Personal information is defined as any information that specifically identifies you or from which your identity can be easily determined.
We collect your personal information primarily to provide our services to you and share information with our clients. We may also use your information for secondary purposes that are closely related to the primary purpose, where you would reasonably expect such use or disclosure.
When we collect personal information, we will, where appropriate and possible, explain why we are collecting it and how we plan to use it.
Use and Disclosure of Personal Information
Your information will only be used to the extent that it informs our staff and helps them provide the required service. Your information will not be used for any purpose outside of AB Occupational Therapy and the service you have requested or are currently receiving.
Where it is reasonable and practical, we will collect your personal information directly from you. However, in some circumstances, we may be provided with information by third parties. In that case, we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.
Consent and Confidentiality
By using our website and providing us with your information, you indicate your consent. We guarantee that we will treat your personal information responsibly and ethically. You are under no obligation to provide us with any of your information.
Access to Your Personal Information
You can request access to your personal information at any time. You may also request that we update or correct any incorrect information. You can access the personal information we hold about you and update or correct it, subject to certain exceptions. If you wish to access your personal information, please contact us in writing.
AB Occupational Therapy will not charge a fee for your access request but may charge an administrative fee for providing a copy of your personal information. To protect your information, we may require identification from you before releasing the requested information.
Information About Other People
If you provide us with information about a third party (such as your clients, other treating professionals, family members, or other parties involved), you must ensure that you are entitled to disclose that information to us and that you have fully complied with the Privacy Act 1988 in relation to the collection, use, or disclosure of that information.
Security of Information
We take the security of your personal and business information seriously. We take all reasonable steps to ensure that all information is treated confidentially, kept secure, protected against unauthorized use, and maintained only for its intended purpose.
Your personal information is stored in a way that reasonably protects it from misuse and loss and from unauthorised access, modification, or disclosure. When your personal information is no longer needed for its intended purpose, we will take reasonable steps to destroy or permanently de-identify it. However, most of the personal information is, or will be, stored in client files that will be kept by us for a minimum of seven years.
